Step 1.

Fill out the Form Below COMPLETELY. Make sure and click on all the links! If this is a telehealth appointment make sure that the phone number you provide is the number you want the doctor to call you on for your appointment.

Step 2.

Don't forget: click on the 4 links and fill out COMPLETELY. This means you'll be completing the 1. Low THC Waiver, the 2. Waiver and Consent Policies, and you will 3. Upload the FRONT and 4. Upload the BACK of your Georgia ID / Driver's License

Step 3.

Get ready for your appointment! Make sure you have access to the phone number on file for teledoc appointments or at the location for your in person appointment 10 minutes early!
